The “2017 Pinnacle Awards:  Innovation in Pharmacy Practice Lecture,” was recorded and presented by HealthPartners representative, Daniel Rehrauer, PharmD, Senior Manager Medication Therapy Program. Titled, "From Medicare to Coverage for All: The Evolution of a MTM Program Over the Decade," this presentation showcases innovation in the continually evolving profession of pharmacy: from the constantly changing pharmacy practice and the corresponding increase in variety and complexity of patient care services offered by pharmacists. 

The Pinnacle Awards, given annually by the APhA Foundation, celebrates three recipients for their significant contributions to the medication use process through increasing patient adherence, reducing drug misadventures, promoting the use of national treatment guidelines, improving patient outcomes, or enhancing communication among the members of the healthcare team.

Learning Objectives

At the completion of this activity, the pharmacist will be able to:

  1. Discuss how delivering comprehensive medication management (CMM) improves patients’ health, provides an excellent care experience, and reduces the total cost of care.
  2. Describe data demonstrating effectiveness of CMM in a variety of areas. 
  3. Explore the effects of a pay for performance program on desired outcomes.
  4. Explain the importance of data standards and the need for CMM documentation platforms.
  5. Develop strategies for starting or expanding a CMM practice.

The American Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education (CPE).
