Join the APhA-APPM Transitions of Care Special Interest Group for Use of telehealth to enhance pharmacy services during transitions of care. This webinar will review telehealth in transitional care pharmacy practice, clinical implications that support pharmacist-led telehealth, federal legislation related to telehealth. The webinar will also address challenges to the implementation of pharmacy-driven telehealth services.
