The Centers for Medicare and Medicaid Services define transitions of care as the movement of a patient from one healthcare setting to another. Medication use, including routine immunizations, is an essential component of pharmacist-driven transitions of care. Many states permit pharmacists and student pharmacists to administer immunizations to patients. Administration schedules, dosing frequency, immunization efficacy, and follow-up care require thoughtful care coordination. Although the Centers for Disease Control and the Advisory Committee on Immunization Practices support pharmacist-administered immunizations, there is less information on the interplay between immunizations and transitions of care.
This archived webinar will offer a relevant and evidence-based discussion on how transitions of care may be leveraged within pharmacist-led immunization practices.

- Identify strategies to improve adherence to immunization schedules.
