Most patients with hypertension easily reach their goal blood pressure with a simple treatment regimen such as a thiazide diuretic and an angiotensin-converting enzyme (ACE) inhibitor. But for some, the hypertension is considerably less responsive to treatment. Resistant hypertension is defined as blood pressure that remains above goal despite concurrent use of three different first-line antihypertensive drug classes. Patients with resistant hypertension are at high risk for cardiovascular events and are more likely than those with controlled hypertension to have a secondary cause, which is usually at least in part reversible.
