The Centers for the Advancement of Pharmacy Education supports student pharmacist education and training within transitions of care as a component of medication use systems management; however, the educational objectives are unclear on how to best deliver that content. Pharmacy literature supports innovations in providing both didactic instruction in Doctor of Pharmacy (PharmD) curriculum as well as experiential learning on introductory and advanced pharmacy practice experiences (APPE).
During this webinar, the speakers will discuss the need for educating APhA members on the best practices to facilitate student pharmacist training in transitions of care education in the classroom and in the experiential settings. This webinar is intended to provide continuing professional education where learners will be able to achieve the course’s learning objectives.
At the completion of this knowledge-based activity, participants will be able to:
- Identify transitions of care PharmD program educational outcomes.
- Discuss strategies to incorporate transitions of care training into the didactic PharmD curriculum.
- Describe strategies for integrating transitions of care activities into PharmD experiential training.
