Pharmacist practice in medical homes and ACOs is expanding at a rapid rate. Value-based payment models are allowing for the sustainability of pharmacy services in these evolving care models. Community pharmacy can bring value to providers, payers, and other stakeholders involved in value-based models. This archived webinar will give SIG members an inside view of the infrastructure changes needed in community pharmacy to participate in and thrive in value-based models.
