Social determinants of health are the conditions that can lead to disparities among patients and barriers to quality patient care. With community pharmacists being the most accessible healthcare providers, there is a huge role for them in overcoming the social determinants of health. In this webinar, a community pharmacy champion who has overcome the challenges associated with social determinants of health will lay out the ground of optimizing pharmacist patient care services. To obtain credit for completing Community Champions: Social Determinants of Health and Pharmacy Practice,  participants will need to watch the archived webinar, complete the assessment with a grade of 70% or better, complete the evaluation questions, and then CLAIM credit.
