With growing interest in value-based payment models, there is opportunity for pharmacists in accountable care organizations (ACOs), patient centered medical homes (PCMHs), and more traditional care sites to provide care for patients. These pharmacists seek to improve patient outcomes and related quality scores while reducing the cost. They practice in a variety of settings, some bill for their services while others see patients with the intent to lower the overall cost of care and improve quality. Many have collaborative practice agreements with their providers and see patients on a referral basis. Whether you are wanting to expand services or are just getting started, this webinar will offer advice from successful practices regarding how to put these strategies into practice.
